Appointments

We are currently accepting new patients! Please call our office at 973-379-2525 to set up an appointment. If for some reason you are unable to make your scheduled appointment time, please call us with at least 24-hours notice to reschedule.

What to Bring to Your First Appointment

• Any relevant medical records
Please arrive to your first appointment 15 minutes early to fill out our patient forms. If you would like to complete this paperwork before your appointment, please print and fill out the forms below.

Insurance

Our office processes most dental insurance plans. We will prepare any necessary forms for your dental benefits. However, please familiarize yourself with your benefit maximums, deductibles, and covered percentages, as your specific policy is an agreement between you and your insurance company.

Patients are responsible for their total obligation if insurance benefits provide less coverage than anticipated. We do not render services based on the assumption the insurance companies will pay our fees–we provide the services that are best for your dental and oral health. Your benefits are contracted between your employer and your insurance company.

Payment

Payment is expected at the time services are rendered, unless you have made other arrangements with us prior to your appointment. This includes any copayments or deductibles. For your convenience, we accept cash, most major credit cards, and CareCredit.
